Register by telephone
It’s easy to register by telephone and many people
find it very convenient. Telephone registration must
be done at least 24 hours before your appointment. Simply
call (434) 517-3862 between the hours of
8:30 A.M. and 4:30 P.M. Monday through Friday. When
you call you might reach a recording asking you to leave
your name, telephone number and the time of day that you
would like us to return your call. Please leave all
the information requested. You will be called back.
Please have the following with you when you talk to our
registration representative:
- All of your current insurance cards.
- The physician order form for the test or procedure you
need. Make sure your doctor’s signature and
the space marked “Principle DX or Reason for Procedure” are
completed. If not complete, you need to notify your
doctor’s office.
On the day of your procedure you may go directly to the
area in which your test or procedure will take place. Our
volunteers will help you find your way. Be sure to follow
your doctor's orders carefully, as some procedures and tests
require certain advance preparations.
