Admissions/Registration
Register in person
To register in person, simply come to the Admitting Office
located on the right, just inside the main entrance of Halifax
Regional Hospital. You may register between the hours of 6:30 A.M. and 5:00 P.M. Monday through Friday. If,
for some reason, you are unable to register during normal
office hours, you may go the Emergency Department and register
there. (Please keep in mind that if the Emergency Department
is busy, you may have to wait.)
When you come to register, please bring the following with
you:
- All of your current insurance cards.
- A photo ID.
- The physician order form for the test or procedure you
need. Make sure your doctor’s signature and
the space marked “Principle DX or Reason for
Procedure” are completed. If not complete,
you need to notify your doctor’s office.
- If you are registering a minor who is not your child,
please make sure you bring the completed “Consent
for Treatment of a Minor” form. Forms can be
picked up from the Admitting Office during regular business
hours, or click here to download a copy.
Once you are registered, our staff will direct you to your
next destination.
