Subacute Unit

What is the subacute unit?

The subacute unit at Sentara Halifax Regional Hospital is a 19-bed skilled nursing facility, licensed by the Commonwealth of Virginia. This special unit is designed to meet the needs of patients who have been in the hospital for at least three days (some insurance policies do not require the three-day stay) who would benefit from additional restorative care, therapies and educational programs before being discharged. Care on the subacute unit is delivered by a multidisciplinary team of clinical professionals with nursing services offered 24 hours a day, seven days a week. Patients admitted to the subacute unit must have an established diagnosis. Most patients utilizing subacute care services stay less than 21 days and are discharged home or to a long-term care nursing facility.

Common diagnoses that benefit from subacute care include:

  • Major joint procedures such as a fractured hip, total hip replacement or total knee replacement
  • Stroke
  • CHF
  • Respiratory problems such as COPD or pneumonia
  • Septicemia/urinary tract infections requiring IV antibiotic therapy
  • Open wounds requiring daily wound care
  • Other diagnoses in which the patient meets admission criteria

In order to qualify for admission to the subacute unit the patient must:

  • Have a skilled nursing or rehab need
  • Have a prior stay in an acute care facility for three days if Medicare is the payer
  • Have an established diagnosis
  • Have potential for functional improvement or have needs that fit skilled care guidelines
  • Expect to be discharged home or to another appropriate facility in 20 days or less
  • Have a discharge summary, admission notes and orders
  • Have a discharge plan established

Services offered on the subacute unit include:

  • Comprehensive nursing care
  • Various therapies and medications
  • Wellness education, including training in illness, injury prevention and self-care
  • Rehabilitative services, including:
    • Physical therapy
    • Occupational therapy
    • Speech therapy
    • Respiratory therapy
  • Nutritional assessment and counseling
  • Home health counseling
  • Activity program
  • Laboratory, radiology and pharmacy services
