Home Health Services

Halifax Regional Health System recognizes that not everyone leaving the hospital is ready to resume a normal routine, so for those needing further help, Halifax Home Health is the answer.

Nursing

Registered nurses provide nursing care and may teach you and members of your family how to provide care that will speed your recovery. Licensed practical nurses provide care under the supervision of a registered nurse. Halifax Home Health is Medicare and Medicaid certified and is accredited by the Joint Commission.

Home Health Aides

Certified aides help with bathing, grooming, dressing and other personal needs.

Therapy

Licensed physical, speech and occupational therapists provide care to aid in recovery from surgery, stroke, developmental problems or other serious medical conditions.

Medical Equipment and Supplies/Laboratory Services

Medical supplies, lab studies and assistance in securing medical equipment is also available.

CareLink

CareMed

CareMed is a home-based, automated medication dispensing system that helps simplify the management of multiple medications, reducing the risk of missed or double doses. Click here to learn more about this system.

CareVital

CareVital is a service that remotely monitors a patient’s blood pressure, body weight and/or blood glucose. Click here to learn more.

On-Call Nurse

A registered nurse is available seven days a week, 24 hours a day.

Admissions Criteria

  • Skilled nursing or therapy need.
  • Physician’s order.

Levels of Care

Minimal care

  • Assistance with personal care such as bathing and dressing.
  • Assessment of cardiopulmonary function.
  • Blood pressure monitoring.
  • Diet instruction.
  • Medication instruction.

Intermediate care

  • Physical therapy.
  • Speech therapy (for example, following a stroke).
  • Occupational therapy.
  • Medical equipment (hospital bed).
  • Medications (visiting nurse to monitor injections, etc.).

Intensive care

  • I.V. therapy.
  • Intravenous feedings.
  • Daily medical monitoring by a nurse.
