Admissions/Patient Access & Registration

Register Online

You can register online here with our secure online form.

Register in Person

To register in person, simply come to the admitting/patient access office located on the right, just inside the main entrance of Halifax Regional Hospital. You may register between the hours of 7 a.m. and 5 p.m. Monday through Friday. If for some reason you are unable to register during normal office hours, you may go the emergency department and register there. (Please keep in mind that if the emergency department is busy, you may have to wait.)

When you come to register, please bring the following with you:

  • All of your current insurance cards.
  • Photo ID.
  • The physician order form for the test or procedure you need. Make sure your doctor’s signature and the space marked “Principle DX or Reason for Procedure” are completed. If not complete, you need to notify your doctor’s office.
  • If you are registering a minor who is not your child, please make sure that you bring the completed “Consent for Treatment of a Minor” form. Forms can be picked up from the admitting office during regular business hours, or click here to download a copy.

Once you have registered, our staff will direct you to your next destination.

Register by Telephone

It’s easy to register by telephone and many people find it very convenient. Telephone registration must be done at least 24 hours before your appointment. Simply call 434.517.3862 between the hours of 8:30 a.m. and 4:30 p.m. Monday through Friday. When you call, you might reach a recording asking you to leave your name, telephone number and the time of day that you would like us to return your call. Please leave all of the information requested and you will be called back.

Please have the following with you when you talk to our registration representative:

On the day of your procedure, you may go directly to the area in which your test or procedure will take place. Our volunteers will help you find your way. Be sure to follow your doctor's orders carefully, as some procedures and tests require certain advance preparations.
